In this white paper the members of the Joint Initiative Council on Global Health Informatics Standardization illustrate their role in enabling the digital transformation of healthcare. The shared vision and way of collaborating among the members of the JIC is presented. To make it concrete, the paper describes four distinctive real-world cases, one of which is the International Patient Summary. As more and more, patients are receiving treatment at locations other than their traditional points of care, the real-world requirement is that their relevant medical information must be available to those caregivers who need it, when they need it and wherever they need it.
